Sub: Unable to claim life insurance policy benefits under the master policy No: JJ000002.

Hello Sir/Madam,
I, Smt. Lalita Laxman Dalwai wife of late Laxman Bhimappa Dalwai, am writing this email regarding the insurance claim under the master policy No: JJ000002
of PMJJBY.

I have submitted required documents on 23rd Aug 2021 to the below mentioned bank claiming policy benefits.

Bank Name: Union Bank
Branch: Kudachi
Taluk: Raibag
Dist: Belgaum
PinCode : 591311

Documents submitted to the bank are as follows:

1) Insurance claim form
2) Death certificate
3) Certificate of insurance on joining
4) KYC of nominee
5) Bank statement of nominee
6) Bank statement of life assured
7) Aadhaar card of life assured

I have received the acknowledgement from Union Bank, Kudachi with reference number PMJJY/1/NK2021. After that I received a rejection letter from Star Union Dai-Ichi life insurance mentioning:

"As per the terms and conditions of the scheme, the maximum age of entry at the time of member enrollment is 50 years calculated based on the nearby birthday as on date of application", and also they suggested we contact the bank for more details.

However based on above mentioned letter, the fault completely lies with the Insurance Company/Bank for following reasons;

1) The Union Bank has accepted the application without verifying the age of applicant at the time of policy purchase.
2) The Insurance company issued policy certificate, thus accepting that all the requirements under the terms and conditions were met by the policy holder.
3) The Union Bank continued to deduct 330 rupees from policy holders A/c : [Protected]every year from date of joining the membership, without verifying the accuracy of the KYC details of the policy holder, including age and date of birth.

Star Union Dai-Ichi life insurance accepted that it was a mistake made by Union Bank Branch at Kudachi. However they are denying me, the nominee, to claim the policy benefits.

The matter is being followed up since 23rd Aug 2021, with the bank and insurance company without any results. Your assistance is requested to settle the claim and get me the legitimate right.

I am prepared to furnish any additional details in the matter, if required by the Insurance Company/Bank. However if the issue is not addressed appropriately
I am prepared to approach the RBI and bring this issue to court.
