Kindly provide the detailed discharge summary mentioning the date of admission, date of discharge, presenting complaints, treatment given during the period of hospitalization and treatment advice on discharge for further processing of the claim. The above requirement by Icici Lombard but Artemis Hospital Gurgaon not provided by the same.
